Dr. Butt is a game pre-pubescent boys play with hold beer bottles. They take the beer bottles to their secret hideout and put their penis in the hole. If one of the boys decides he doesn't want to play Dr. Butt anymore he will shunned and forced to make new friends elsewhere.
